Location: Plainsboro, NJ - HYBRID ( 2-3 days a week remote and 2-3 in-person)

PA residents will need to obtain a Social Worker NJ License before employment.*

Hours: Day Shift / 40 hours per week/ No weekends/Holidays - 8 am - 4:30pm - Mon - Fri

Summary:

  • The Oncology Social Worker (OSW) provides psycho-social care to cancer patients and their families across the continuum of the cancer experience from diagnosis through treatment and survivorship and/or palliative and end-of-life care. The OSW is responsible for providing assessment, counseling education, advocacy, and support programs to enhance patient's and families' ability to cope with physical, emotional, environmental, and social problems. The OSW collaborates with the multidisciplinary team to provide compassionate, patient-centered care.

Responsibilities:

  • Provides psycho-social assessments, treatment planning and appropriate interventions or referrals for oncology patients with social, environmental, emotional, financial and/or medical needs.

  • Utilizes cognitive behavior therapy (CBT) in providing individual counseling and facilitating support groups. Recognizes, treats, monitors and refers to appropriate resources when patient experiences distress throughout the disease trajectory

  • Provide crisis intervention when needed.

  • Offers counseling to caregivers and family members of oncology patients as needed.

  • Advocates for and protect patients' dignity, confidentiality, rights, and access to care.

  • Acts on patient’s behalf and best interest among healthcare providers, family, and community.

  • Provides information and resources relative to POLST forms, advanced directives (living wills and powers of attorney) DNR, and organ and tissue donation, and assists in documenting forms in the patient’s electronic health record.

  • Assesses patients and families relative to needs and provides linkages to community resources and governmental programs available from local, regional, and national health and welfare agencies.

  • Provides patient and family with referral(s) to resources: financial, spiritual, social and emotional support programs.

  • Works in collaboration with other social workers to establish and maintain a financial counseling program.

  • Identifies uninsured and underinsured patients and serves as a patient advocate with Medicare, insurance companies, pharmaceutical companies and other assistance programs.

  • Communicates with patients, caregivers, navigators, ordering physicians, and other members of the treatment team, financial implications of medical intervention on both the patient and the health care provider.

  • Educates patients regarding insurance coverage and out-of-pocket expenses

  • Estimates patients’ financial obligations for services.

  • Participates in multidisciplinary conferences such as Tumor Board and Cancer Committee meetings.

  • Responds to patient/family and care team in a timely manner and documents appropriately.

  • Upholds professional standards by adhering to the NASW code of ethics and AOSW standards of care for oncology social workers.

  • Prepares and presents educational activities to patients/families, and multidisciplinary groups within PMPH as well as the local and regional community.

  • Supervises Social Work students from affiliated universities as needed.

Credentials:

  • Licensed Clinical SW (Required)

  • Active Clinical Social Worker License (LCSW) in the state of New Jersey

  • Cert Oncology Social Worker

  • Oncology Social Work Certification (OSW-C)

Education or Equivalent Experience:

  • Master of Arts or Science (Required)

  • Education Specialization:Master of Social Work from graduate school of social work approved by the Council on Social Work EducationEquivalent

  • Experience: •And 2+ years Hospital social work or related health care experience•Experience with geriatric populations
